Clinical Team
The WOTCH Clinical Team enhances existing services at WOTCH by increasing the emphasis on health promotion, disease and injury prevention and chronic disease management. The Clinical Team's goal is to offer services that promote an individual's overall health and well being and improve access to primary health care and psychiatric services in the community. Clinics and groups are offered to individuals as well as one-on-one assessments and interventions that include:
- Consultation Clinic with a psychiatrist
- Medical Clinic with a family physician
- Diabetes screening and self management program
- Free Foot care Clinics
- Collective Kitchen
- Nutrition Clinic
Occupational Therapy: At WOTCH the occupational therapists work to help individuals to succeed at activities they want or need to do in their chosen environments. Based on the client's goals, the occupational therapist will assess components of the activity, the person, and the environment. Individual and group interventions may be directed at making adjustments in these domains to create a fit more conducive to success.
Health Promotion: Nurses provide ongoing health teaching through feedback, explanations and rationale for diagnostic tests, medications, procedures and treatments. Our goal is to assess and maximize the ability of an individual to take responsibility for and manage their health needs within available resources and their personal skills. Individuals are supported in healthier choices.
Crisis Management: Clinical Team is available to assess the physical health and psychiatric needs of an individual and will assist in making appropriate referrals to community physicians and support services.
Team Approach: Through a multidisciplinary approach the intervention plan is promoted to collaborate and support an individual in meeting personal goals.
Psychosocial Rehabilitation and Recovery: The clinical team assists clients in identifying their strengths and available resources and supports clients in the development of self-advocacy skills.
