Intensive Case Management Services

WOTCH Intensive Case Management Services are voluntary and provided within a framework of psychosocial rehabilitation and recovery. Each client referred for intensive case management services will have a Case Manager (CM) assigned.   • Assessment: The Camberwell Assessment of Need is used as the starting point to identify specific client needs at the time of referral. During the initial period of engagement additional assessments are completed and used as the basis for developing a rehabilitation and recovery plan with the client.
  • Individualized Rehab Plan Development: Based on the assessment, the CM and client develop an individualized rehabilitation plan which identifies specific goals and strategies to achieve these goals. The six areas of focus may include: Daily Living, Financial Management, Health, Leisure/Recreation, Vocational/Education and Relationships. Also included is the establishment of a crisis plan and safety plan when indicated.
  • Direct Service Provision: The CM meets with the client regularly as outlined in the rehabilitation plan to assist them in developing the skills and supports necessary to effectively manage their illness and live successfully in the community.
  • Service Coordination and Advocacy: The CM works with the client, family, other WOTCH programs and community agencies to advocate for and coordinate services that are appropriate and will assist the client in achieving their goals.
  • Monitoring and Evaluation: The CM monitors and reviews progress regularly with the client to ensure that the services provided are appropriate to their needs and goals. Changes to the individualized rehabilitation plan are made as needed.
